Any client who is seeking treatment at Sojourn House must first be assessed.
There is no client charge for an adolescent assessment. However, if you have Medicaid or insurance, please bring your card with you to your appointment.
Payment is required for all DUI evaluations/updates at the time of the appointment. The cost is $130. Anyone who is seeking a reduced fee for a DUI evaluation must review eligibility for a reduced rate with the business office prior to their appointment. No reduced rates are available for DUI updates.
The cost for an adult assessment is $150. If you have insurance, make sure to bring your card with you to your appointment.
Reduced rates are available. If you have Medicaid, make sure to bring your card with you to your appointment. All other clients should bring proof of income to determine their eligibility for a reduced rate.
Please be aware that the assessment and DUI evaluation/update processes for adults and adolescents could require you to be at our facility for up to three hours, so please plan accordingly.
Residential Rehabilitation Treatment
If you are seeking residential rehabilitation treatment at our facility, the following will outline the process that you will need to follow in order to get admitted into our program.

You should arrive at our facility between 8:30 and 9am Monday through Friday, or between 11:30am and 12pm on Friday, to be assessed. Please come prepared for admission to our facility that same day. You will need to make sure you have the following items with you:
No CDs, IPods, MP3 players, TVs, or computers are allowed in residential treatment. NO outside food, drink, or candy is allowed. Also do not bring in over the counter medications as you will not be permitted to have them here. You will also not be permitted to leave a vehicle here once admitted so plan to have someone come with you.
If you are on medication or are experiencing a psychiatric or medical condition…
You should arrive for an assessment on a Monday or Tuesday morning only between 8:30 and 9am to be assessed. Please come prepared for admission to our facility that same day. Please make sure to bring the items listed above, as well as 30 days worth of each of your medications in their original bottles. If you are unable to bring in a 30 day supply of those medications, make sure to bring your medical card, insurance card or money to pay for refills of those medications. Please be aware that all medications must be reviewed by our nurse and doctor to determine if they are appropriate to be taken at our facility.
Our goal is to directly admit every client that is seeking residential rehabilitation treatment. However, please be aware that there is no guarantee that we will be able to do so for every client.
Residential Extended Treatment (Halfway House)
Clients seeking Residential Extended Treatment (Halfway House) should follow the same procedure as is outlined for Residential Rehabilitation Treatment.
Cell phones may be used by Halfway House clients and CDs, IPods, and MP3 players are also permitted.
Adult Outpatient Treatment
Clients seeking our intensive outpatient program, outpatient treatment, or have been referred for an adult assessment, should arrive only on a Wednesday, Thursday, or Friday between 8:30 and 9am, or Friday between 11:30am and 12pm, to be assessed.
Adolescent Outpatient Assessment/Treatment or DUI Evaluation/Update
Clients who seeking adolescent outpatient or DUI treatment, or have been referred for an adolescent assessment or DUI evaluation/update, will need to call to set-up an appointment time.
